These numbers are an estimate, drawn from UK pricing sources including NHS dispensing data, not a quote from any specific pharmacy. Actual prices vary by pharmacy, pack and location, so treat the numbers as a starting point, not a promise. A price well below the listed range should be treated as a red flag about the seller's license.

This is a prescription-only medicine (POM). A valid UK prescription from a UK-registered prescriber is required before a pharmacy can dispense it.

Fill it at a pharmacy registered with the GPhC (Great Britain) or the PSNI (Northern Ireland).

Ranolazine is a POM under UK medicines law under UK medicines regulation. A UK pharmacy can only dispense it against a valid prescription from a UK-registered prescriber.

If you do not have a current prescription, contact your GP surgery or a prescriber registered in the UK before the pharmacy can proceed. Expect the pharmacy to check that the prescriber is UK-registered, as a normal part of dispensing a prescription.

About Ranolazine

Ranolazine is an antianginal medicine used for chronic angina, a condition that causes chest pain or discomfort when the heart muscle does not receive enough oxygen. It is available as an extended-release oral medicine and has been available as a generic since 2013-07-29. In the United Kingdom, it requires a prescription.

What Ranolazine is used for

Ranolazine extended-release tablets are indicated for the treatment of chronic angina. Chronic angina can cause recurring chest pain or discomfort because the heart muscle does not receive enough oxygen. Ranolazine may be used alongside beta-blockers, nitrates, calcium channel blockers, antiplatelet therapy, lipid-lowering therapy, ACE inhibitors, and angiotensin receptor blockers.

The medicine is intended for ongoing control of chronic angina rather than treatment of diabetes. A large clinical trial in acute coronary syndrome did not demonstrate a benefit for ranolazine.

Dosage from the label

The label lists ranolazine as oral granules of 1000 mg and as extended-release tablets of 500 mg and 1000 mg, including film-coated extended-release tablets. The usual starting dose is 500 mg twice daily. The label lists an increase to 1000 mg twice daily when needed according to clinical symptoms.

The label lists 1000 mg twice daily as the maximum recommended daily dose. The label lists a limit of 500 mg twice daily when ranolazine is used with moderate CYP3A inhibitors such as diltiazem, verapamil, or erythromycin.

The label states that tablets are used with or without meals and swallowed whole, not crushed, broken, or chewed. The label states that a missed dose is followed by the prescribed dose at the next scheduled time, without doubling the next dose.

How Ranolazine works

Ranolazine is an antianginal medicine. It helps the heart muscle work more efficiently when oxygen supply is limited. Its main action is thought to involve reducing a late flow of sodium into heart muscle cells.

This can reduce the build-up of calcium inside those cells and ease some of the tension placed on the heart wall. By reducing this cellular stress, ranolazine can help control symptoms of chronic angina without depending mainly on lowering heart rate or blood pressure. Ranolazine can also prolong the QTc interval, so its effects on the heart’s electrical activity need to be considered, particularly alongside other medicines that prolong the QT interval.

Main interactions

  • Strong CYP3A inhibitors such as ketoconazole, clarithromycin, and ritonavir must not be used with ranolazine
  • CYP3A inducers such as rifampin, carbamazepine, and St. John's wort must not be used with ranolazine
  • Diltiazem, verapamil, erythromycin, fluconazole, and grapefruit juice can require a ranolazine limit of 500 mg twice daily
  • Cyclosporine and other P-gp inhibitors can increase ranolazine exposure
  • Ranolazine can increase exposure to digoxin
  • The simvastatin dose is limited to 20 mg once daily with ranolazine
  • Ranolazine can increase exposure to tricyclic antidepressants and antipsychotics
  • Ranolazine 1000 mg twice daily can increase metformin levels, with metformin not exceeding 1700 mg daily

Key warnings

  • Ranolazine can prolong the QTc interval
  • Use is contraindicated with liver cirrhosis
  • Use is contraindicated with strong CYP3A inhibitors
  • Use is contraindicated with CYP3A inducers
  • Renal function requires monitoring in moderate to severe renal impairment
  • Acute renal failure has been observed in severe renal impairment
  • Discontinue ranolazine if acute renal failure develops
  • Dizziness, headache, constipation, and nausea are common adverse reactions

Pregnancy and breastfeeding

There are no available data on ranolazine use in pregnant women to inform drug-associated risks. Studies in rats and rabbits showed no evidence of fetal harm at exposures 4 times the maximum recommended human dose, although decreased fetal weight and reduced ossification occurred in rats at exposures associated with maternal weight loss.

There are no data on ranolazine in human milk, effects on a breastfed infant, or effects on milk production. Pregnancy or breastfeeding should be discussed with a doctor or pharmacist.

Missed dose

A missed dose means the prescribed dose is omitted until the next scheduled time. The label states that the next scheduled dose should be used and that the next dose should not be doubled.

Regular timing helps maintain ongoing control of chronic angina. A pharmacist or doctor can provide advice if missed doses happen repeatedly or if angina symptoms worsen.

Storage

Store at 20°C to 25°C. Excursions are permitted between 15°C and 30°C. Keep the medicine protected as directed on the label, and do not use it if the blister is torn or broken.
