What Ropinirole is used for

Ropinirole is indicated for Parkinson's disease and moderate-to-severe primary restless legs syndrome. For Parkinson's disease, the label describes use both in early disease without levodopa and in advanced disease alongside levodopa.

For restless legs syndrome, the indication is specifically for the primary form of the condition when it is moderate to severe. These conditions have different treatment schedules, so the prescribed regimen depends on the reason for treatment.

Dosage from the label

The label lists film-coated tablets containing 0.25 mg, 0.5 mg, 1 mg, 2 mg, 3 mg, 4 mg and 5 mg. The label lists extended-release tablets containing 2 mg, 4 mg, 6 mg, 8 mg and 12 mg. The usual starting dose is 0.25 mg three times daily for Parkinson's disease.

How Ropinirole works

Ropinirole belongs to a group of medicines called dopamine agonists. Dopamine is a chemical messenger that helps nerve cells communicate, including in brain pathways that control movement. Dopamine agonists stimulate the receptors that normally respond to dopamine, helping support signalling in these pathways.

Ropinirole acts like dopamine at these receptors rather than being converted into dopamine. This action explains its use in Parkinson's disease and restless legs syndrome.

There are no adequate data on developmental risks in pregnant women. Animal studies suggest ropinirole may harm an unborn baby. There are no data on its presence in human milk or its effects on breastfed infants or milk production.

Reduced milk production is expected because ropinirole lowers prolactin secretion. Discuss pregnancy and breastfeeding with the prescriber, weighing treatment needs against potential risks.

Missed dose

A missed dose can interrupt symptom control. Do not double a dose to make up for a missed one.

Check the patient leaflet or ask a pharmacist about the prescribed schedule. After a significant break in treatment, contact the prescriber before restarting, as gradual dose adjustment may be needed again.
