About Telmisartan

Telmisartan is an angiotensin II receptor blocker (ARB), most commonly prescribed to treat high blood pressure. Lowering blood pressure helps reduce the risk of cardiovascular events, particularly strokes and heart attacks, and telmisartan may be used alone or alongside other blood pressure medicines. It requires a prescription in the United Kingdom and is available as a generic medicine.

How Telmisartan works

Telmisartan belongs to a group of medicines called angiotensin II receptor blockers, or ARBs. Angiotensin II is a chemical signal in the body that narrows blood vessels and helps regulate salt and fluid balance. Telmisartan blocks the receptors through which this signal acts, allowing blood vessels to relax and helping lower blood pressure.

This reduces the pressure against which the heart pumps. Its effect on this hormone system also explains why kidney function, blood pressure and potassium levels may need monitoring during treatment.

What Telmisartan is used for

Telmisartan treats hypertension, the medical term for high blood pressure. Lowering blood pressure reduces the risk of strokes, heart attacks and death from cardiovascular causes. It may be used alone or alongside other medicines that lower blood pressure, as part of wider cardiovascular risk management.

The label also lists cardiovascular risk reduction for people aged 55 years or older who are at high risk of major cardiovascular events and cannot tolerate ACE inhibitors. This includes people with a history of coronary artery disease, peripheral arterial disease, stroke or transient ischaemic attack, or high-risk diabetes with organ damage. In this setting, telmisartan is intended to reduce the risk of heart attack, stroke or death from cardiovascular causes and may accompany other necessary treatments.

Dosage from the label

The label lists oral tablets in strengths of 20 mg, 40 mg and 80 mg. The usual starting dose is 40 mg orally once daily for hypertension. The label lists a hypertension dose range of 40 to 80 mg once daily, with treatment individualised according to blood pressure response.

Liver impairment requires cautious dose selection and gradual adjustment. When treatment begins for cardiovascular risk reduction, blood pressure monitoring and adjustment of other blood pressure medicines may be necessary.

The patient leaflet or a pharmacist can clarify whether a forgotten dose should be omitted when the next scheduled dose is approaching.

Pregnancy and breastfeeding

Telmisartan can harm an unborn baby, particularly through effects on kidney function during the second and third trimesters. The label calls for discontinuation as soon as pregnancy is detected.
