About Testosterone
Testosterone is an androgen hormone medicine used mainly as replacement therapy in males whose bodies produce too little or no testosterone. Testosterone cypionate is indicated for conditions involving testicular failure or deficient hormonal signals controlling testosterone production; testosterone requires a prescription in the United Kingdom, and generic versions are available.
How Testosterone works
Testosterone belongs to the androgen hormone class. Androgens bind to receptors within cells and influence the activity of genes involved in male sexual development and the maintenance of androgen-dependent tissues. Testosterone replacement supplies hormone when the body does not produce enough naturally.
Its effects involve reproductive tissues, muscle and bone, as well as other body systems. Androgens also participate in hormonal feedback between the brain and testes, so treatment can affect the body's own hormone signalling and sperm production.
What Testosterone is used for
Testosterone cypionate injection is indicated for replacement therapy in males with symptoms associated with deficient or absent natural testosterone. This includes primary hypogonadism, where the testes fail to produce enough testosterone because of congenital or acquired conditions, including undescended testes, inflammation, torsion or surgical removal. It also includes hypogonadotropic hypogonadism, where deficient hormonal signals or damage to the pituitary gland or hypothalamus affect testosterone production.
Such damage may follow tumours, trauma or radiation. Safety and effectiveness in men with age-related hypogonadism, sometimes called late-onset hypogonadism, have not been established.
Key warnings

- Children may develop accelerated bone maturation and reduced final adult height; benzyl alcohol can cause serious toxicity in paediatric patients
Dosage from the label
The label lists intramuscular testosterone injections at 100 mg/mL and 200 mg/mL, alongside other testosterone formulations including gels, subcutaneous injections, pellets and topical solution. The label lists testosterone cypionate replacement dosing for hypogonadal males as 50 to 400 mg every two to four weeks. Testosterone cypionate injection is intended only for intramuscular administration, deep into the gluteal muscle, and not for intravenous administration.
Before treatment begins, the diagnosis requires morning serum testosterone measurements on at least two separate days, with results below the normal range. Treatment is individualised according to age, sex, diagnosis, response and adverse reactions.
For induction of pubertal changes, the label describes differing approaches to initial treatment and subsequent adjustment, with chronological and skeletal age considered. Injectable solutions require visual inspection for particles and discolouration before administration when the solution and container permit this.

Storage
The label specifies storage of testosterone cypionate injection at 20° to 25°C, protected from light. Keep the medicine in its original packaging and out of the sight and reach of children.
Missed dose
A missed testosterone treatment may interrupt the intended replacement schedule. Advice depends on the formulation and prescribed regimen.

Pregnancy and breastfeeding
Testosterone is contraindicated during pregnancy. It is teratogenic, meaning it may cause fetal harm, and can cause development of male characteristics in a female fetus.
The preservative benzyl alcohol can cross the placenta. The label does not specify lactation data; breastfeeding should be discussed with a doctor or pharmacist.
